How to claim your clothing pack
Option 1
1. Attach a copy of your original product sales receipt to this form.
2. Complete the following clothing redemption registration.
3. Pay for shipping

1. Complete the following clothing redemption registration
2. Pay for shipping
3. Writing your order number (found on the order confirmation page) onto the original proof of purchase receipt.
4. Print out your shipping receipt from the confirmation email that is sent to you at completion of this registration.
5. Email, fax or mail the shipping receipt together with the proof of purchase (an original copy of your product sales receipt) no later then 30 days from purchase date.
